Welcome to the CAIR New Organization/Site Enrollment Page!
NOTE: This website is for Organizations that would like to begin using CAIR or for Organizations already using CAIR that would like to enroll a new site. If your Organization is already using CAIR and you would like to request user accounts for new staff, change user accounts for existing staff, or make other changes to your Organization's information in CAIR, go to the CAIR Account Update website at http://accountupdate.cairweb.org/.
Before beginning the new Organization/Site registration process, do the following:
- If you intend to submit patient immunization information to CAIR electronically (e.g., via your EHR), enroll at the CDPH Gateway/IZ Portal instead. Do not enroll through this website.
- If your Organization already has a CAIR Account, do not enroll here. Instead go to Account Update at http://accountupdate.cairweb.org/
- If enrolling as a ‘clinical’ Organization/Site (e.g., your Org/Site gives immunizations), have your Responsible Clinician’s Full Name and CA Medical License Number available.
- If you are a VFC provider please have your VFC PIN ready and indicate if you receive VFC, 317, and/or State General Fund vaccine.
NOTE: You must complete the entire enrollment application; you cannot save the information and return to it later. If you exit the application before clicking the ‘Submit’ button, all information will be lost. The application should take about 15 minutes to complete.
Once you submit your new Organization/Site enrollment please allow up to 10 business days for your Application to be reviewed and processed.

*Providers in Alpine, Amador, Calaveras, Imperial, Mariposa, Merced, San Diego, San Joaquin, Stanislaus, and Tuolumne counties will not be able to use online enrollment. Click here for contacts in those counties.
